Doctor Notes Submissions
To ensure we have accurate records and comply with our health policies, parents are required to submit a doctor’s note whenever their child is absent due to illness. Please use the form provided on this page to upload the doctor’s note promptly. This helps us maintain a safe and healthy environment for all children and staff. We appreciate your cooperation and commitment to our community’s well-being.

Dropping a Child Notice
As outlined in our parent handbook, a two-week notice is required to withdraw a child from our program, regardless of the situation. We recommend that your child attends during these final two weeks to ensure you receive the full value of your payment.
If you are a DHS client, you will still be responsible for the full amount of the two-week period, even if your child does not attend. Once a drop notice is submitted, your child will remain in our care for an additional 14 business days, and your final payment will be automatically processed two Thursdays/Fridays from the submission date. This does not include the current week’s payment, which is separate. If you decide to continue with our services after submitting a drop notice, it will be at our discretion whether we can accommodate your child.

DHS Recertification Document Requests
To ensure uninterrupted childcare services covered by DHS, parents must complete recertification required by DHS. This process helps maintain your eligibility for DHS assistance and guarantees that your child continues to receive the care they need.
